_ oEssential Functions The Licensed Practical Nurse (LPN) in Home Health is responsible for providing quality patient care and performing technical skilled care in the patient's home.
The Licensed Practical Nurse (LPN) practices use of good communication skills, both verbally and written.
Is responsible for adhering to all LPN Nursing Practice Standards as they apply to patient care.
oProvides services within the scope of practice as defined by the state laws governing the practice of nursing, in accordance with the plan of care, and in coordination with other members of the health care team oMay not alter the plan of care oAs assigned, visits patient and provides nursing care according to plan of care oParticipates in the in-service program according to our policy oCompletes a progress note for each visit, and submits progress notes to the agency daily oParticipates in the QA/QI plan and process oContinually observes and reports the patients nursing needs oInstructs the patients family in the care of the patient and maintenance of a healthy environment for the patient oObserves, records, and reports to the supervising nurse and/or physician the patient's response to treatment and changes inthe patient's condition oAdheres to and participates in the Care Management process oParticipates in staff conference regarding patient care oPerforms other duties that may be assigned.
oExperience Requirements One year of clinical experience.
oLicense Requirements o Current LPN licensure in the state of practice Current CPR Certification Current driver's license, valid vehicle insurance, and access to a dependable vehicle, or public transportation.
